4/17/2023 EnchantaVaughn Approved 1. Behavior: Upon arrival to the home setting, QMHP-C engaged Ms. Hodge in a discussion surrounding new behaviors Hodge has been presenting. Ms. Hodge discussed with QMHP-C Hodge has been making some progress since she has been receiving services, but there is a lot of work to be done with her attitude and the way she interacts with those who are in authority. 2. Behavior: QMHP-C entered Hodge’s appeared to be in a positive mood evident by her facial expressions such as a smile on the face. Hodge participated in the session and provided discussion to what was being communicated with QMHP. 1. Intervention: QMHP-C discussed with Hodge the purpose of the session and reviewed expectations of the session that are to be followed. QMHP-C encouraged Hodge to freely and openly express any issues, questions or concerns she may have regarding sessions. QMHP-C discussed with Hodge what she feels is the problem between her mother and herself regarding their communication interaction with one another. QMHP-C utilized a skill building activity to assist Hodge with practicing effective and appropriate communication. QMHP-C enlisted feedback from Hodge and her mother with activity regarding their communication. 2. Intervention: QMHP-C modeled appropriate communication and facilitated a dialogue between parent and child that would assist them in avoiding conflicts and arguing. QMHP-C encouraged Hodge and her mother to be mindful of the need for respect in the parent-child relationship. QMHP-C held Hodge accountable for the inappropriate manner in which she addresses her mother and talks to authority figures. QMHP-C listened attentively and offered support as Hodge shared examples of situations. QMHP-C encouraged Hodge and her mother to be open to changing the way they communicate with one another since the current style is resulting in arguing and conflict. 1. Response: Hodge actively engaged in conversation with QMHP-C during session. Ms. Hodge reported that the Hodge became aggressive with her verbally for the first time she can remember. She reported that the Hodge used profanity toward her. Hodge expressed to QMHP-C that she gets tired of arguing with her mother so now she goes to her room. 2. Response: During the session, Hodge shared that she has begun writing short stories about different events that have taken place in her life and that helps her stay out of trouble by avoiding arguments with her mother. Hodge reported that she journals to avoid expressing herself to others which she has always struggled with doing. Hodge admits that she still does ignore her mother at times, but she eventually does what she asks her to do.
